I acknowledge that participation in wrestling and related activities at Ignite Wrestling Club at Moon Valley High School involves inherent risks, including but not limited to: sprains, strains, fractures, concussions, skin infections, and other injuries. I voluntarily assume all risks associated with training, practice, competition, travel, and related activities, whether supervised or unsupervised.

In consideration of participation, I hereby release, discharge, and hold harmless Ignite Wrestling Club, Moon Valley High School, Glendale Union High School District, their coaches, staff, volunteers, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or loss sustained during participation in wrestling activities.

In the event of an injury or emergency, I authorize Ignite Wrestling Club staff, coaches, or volunteers to obtain medical treatment deemed necessary for my child. I accept financial responsibility for any medical care and related expenses incurred as a result of participation.

I understand that Ignite Wrestling Club does not provide medical insurance for participants. Proof of personal medical insurance is required for participation, and I confirm that my child is covered under an active policy.

I grant Ignite Wrestling Club permission to photograph or record my child during practices, competitions, or events, and to use such media for promotional, educational, or informational purposes without compensation.

I understand that all athletes are expected to follow team rules, respect coaches, teammates, opponents, and officials, and demonstrate good sportsmanship at all times. Violation of the code of conduct may result in suspension or dismissal from the program.

This agreement shall be governed by the laws of the State of Arizona.

By signing below, I acknowledge that I have read and understand this waiver, and I voluntarily agree to its terms.
